At about 3:00 p.m. on January 14, 2022, an employee was trying to place a piece of wood (2x4) under a switchgear, which another employee was holding up with a pry bar. The pry bar slipped and the gear fell onto the first employee's left index finger. The employee suffered a distal amputation of the finger.
