An employee disconnected the cam lock for a forming machine air line. The high-pressure hose struck the employee on the right side of the head. The employee suffered five broken left-side ribs, a fracture and laceration at the right temple, and a second laceration to the head.

An employee was operating a forklift to travel between buildings. As the lift was moving down the ramp, they reached out to press the open button for the overhead door. While pulling their arm back into the lift, the lift started to move down the ramp, catching their arm between a bollard and the forklift frame resulting in a left radius and ulna fracture.
An employee was checking samples of cups for quality in a building without air conditioning. The employee developed dizziness and cramps in the right leg and toe and was hospitalized, suffering from dehydration and heat exhaustion.
