Federal OSHA recorded a severe workplace injury at Custom Fiberglass Installations, LLC., 205 New Castle Avenue, DELAWARE CITY, DELAWARE 19706 on — Fractures, affecting the chest, except internal location of diseases or disorders.

An employee and three co-workers were working to cover a second-story deck with a fiberglass waterproofing membrane. A two-by-four wooden plank was placed between the second-story deck under construction across to an adjacent second-story deck to walk back and forth between the decks via the plank. The employee attempted to walk the plank when the plank snapped and he fell approximately 10 feet to the ground, striking his chest on lower deck framing before hitting the ground. The employee suffered a chest injury and broken ribs.

On December 15, 2023, the injured employee was working from a ladder. A co-worker climbed the backside of the ladder and the ladder failed. The injured employee fell approximately 8 feet to the ground and sustained multiple fractures.

Structural Precast Erectors, LLC

An employee was installing grout using a shovel when the precast hollow-core floor gave way, causing them to fall approximately 8-10 feet. The employee sustained a back fracture.

CTX Home Improvement

An employee was assisting with assembly of a two-level scaffold. The scaffold fell, causing the employee to fall 8 feet from the second level to the concrete surface below. The employee sustained fractures to the face, jaw, and both wrists.

An employee was repairing an egg collector when the platform collapsed and he fell 10 feet to the concrete floor. The employee sustained a fractured left leg.

An employee was descending a ladder when it collapsed, causing him to fall approximately 8 feet. The employee was hospitalized with a compound fracture to his left leg and a left wrist fracture.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was closing a rear trailer door in the parking lot when they were struck by a forklift. The employee's lower left leg was crushed between the pallet on the forklift and the trailer. The employee sustained muscle damage to their lower leg.

An employee was cutting wood with a circular saw. The machine lagged and cut the employee's finger. The employee required surgical amputation of the finger at the hospital.

An employee was climbing a 12-foot ladder when he lost his balance and fell from the eighth rung to the ground. He sustained multiple broken ribs, which caused lung punctures.

An employee was installing spray-on fire proofing from a scissor lift. The scissor lift tipped over and the employee fell, resulting in multiple fractures to their leg, shoulder, and skull.

Greggo & Ferrara, Inc.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

GREGGO AND FERRARA, INC.

An employee was troubleshooting a power washer in the field. Because there might have been water in its fuel, he brought it back to the shop and drained about a gallon of fuel from the tank into a plastic container. Some of the fuel spilled onto the floor and ignited. The employee was stomping out the fire when he lost his balance and tripped into a stool, which caused the plastic container to spill more fuel onto the fire. The employee's pants and shirt caught on fire, and he fell, abrading his knee while trying to get through the flames. As well as the knee abrasion, he suffered burns to the left leg and left lower quadrant of the torso. He was hospitalized.

An employee was backing up a tram (towing powered industrial equipment) to connect it to a trash bin. The employee's left forearm/wrist was caught and crushed between the tram and the bin. The employee was hospitalized.

An employee went to lift a carton and tripped on a different carton that was on the floor. She fell on the floor in the backroom and sustained a fractured right hip, and abrasions to her arm and knee. The employee was hospitalized and required surgery.
