An employee was using a circular saw to cut plywood when the saw kicked back and struck the employee's left finger, resulting in a fingertip amputation behind the nail.
An employee was on a scissor lift while marking a ceiling with a tool so they could cut control joints. The counterweight of a boom lift struck the scissor lift as it rotated, knocking the scissor lift over. The employee fell 14 feet. The employee sustained a femoral neck fracture to their left femur, fractures to three vertebrae, a pelvic fracture, a fracture and dislocation to their left wrist, and scrapes to their left elbow and wrist. The employee was hospitalized.
An employee was cleaning coal build-up inside pulverizers that were 10 feet 3 inches wide and contained three stationary rollers.
