An employee was unloading pallets of product from a trailer when a plastic-wrapped pallet became stuck against the bulkhead of the trailer. The employee was dislodging the pallet when he lost his grip and fell backward, striking his head on the side of the trailer. He then tripped over the forks of a pallet jack and fell to the floor of the trailer. The employee was hospitalized with a brain bleed and pain in his lower back and ribs.

An employee was cutting boxes with a safety knife. The safety guard on the knife broke off and the knife lacerated his left forearm. He was hospitalized.

An employee was on a catwalk five feet above the ground to see into a refrigerator trailer engine. As the employee was adjusting the belt on the engine, they fell off the catwalk to the concrete below, resulting in a left hip fracture. The employee was hospitalized.

An employee was operating a stand-up forklift in the warehouse when it struck another forklift. His left foot was caught between the two forklifts. The employee was hospitalized with a foot fracture and lacerations that required surgery.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.
