An employee was operating a lift truck. He was stopping the vehicle to prevent it from hitting a parked unit and his leg got caught in between the lift truck and a metal pole. The employee suffered multiple leg fractures. He was hospitalized, requiring surgery.

An employee was helping to remove the clamp attachment paddles from a forklift so they could then repair a hydraulic leak. The clamp fell and pinned the employee's right ring finger against the mast. The employee's finger was partially amputated.

On February 22, 2022, an employee was operating a stand-up forklift in the receiving area when their left leg struck a concrete bollard, resulting in a fracture.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
