An employee was backing up his truck to an empty trailer. He then stopped, exited the cab, and walked toward the back of the trailer for inspection. On the way back to the cab, he slipped and fell on ice and snow. The employee sustained broken bones in the left foot and a dislocated left ankle, requiring hospitalization and surgery.

On April 23, 2024, at approximately 7:30 PM, a driver was picking up a trailer at a rail yard. The employee was using a crowbar and hammer to adjust the pins of the chassis to allow the intermodal container to sit on the chassis. The container shifted and the employee's left hand was caught between the container and chassis, resulting in a partial amputation of their middle, ring, and little fingers.

He was trying to pull down the door, gripping a strap with one hand and the trailer grip handle with the other hand, when he fell to the ground 2-3 feet below.
