An employee was cutting a 1-by-6 board on a table saw. The saw amputated his left index finger at the knuckle.
An employee was flipping over a precast form pan to remove the precast concrete product when the form pan fell and fractured his leg just above his steel-toe boot. He required hospitalization and surgery.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
