On February 15, 2022, an employee was adjusting storage racks from a scissor lift when the scissor lift battery died. The employee then climbed onto a steel rack and fell approximately 6 feet from the rack to the concrete floor below. The employee sustained fractures to his leg requiring surgery.

Bunn-O-Matic Corporation

An employee was using a pneumatic pincer to secure an ear clamp onto a juice hose fitting. The employee inadvertently placed the pincer head on the tip of their right little finger and activated the crimper, which pinched their fingertip. The employee sustained a partial amputation of their right little finger.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was removing product (a transformer radiator weighing approximately 2,640 pounds) from the paint room utilizing an overhead drop-crane system. The employee inspected the product in the drop section and started to lower the load. The product fell out of the rigging and hit the ground. It then tipped, pinning the employee against a desk. The employee was hospitalized with collapsed lungs, a dislocated sternum, a head injury requiring staples, and fractures to their ribs and wrists/lower arms.

An employee was retrieving an empty blue bin. When he turned around, he tripped over a skid and fell to the floor, striking his head and sustaining injuries to his hip and foot. The employee was hospitalized with a fractured left hip.
