An employee was working with a team to install fiber cable. While the team was pulling the cables and laying them in place, he began cramping and felt overheated. He was hospitalized, suffering from heat exhaustion.

An employee had been working outside during the day on April 11, 2022, where the temperature reached a high of 89 degrees. The employee drove to a gas station where he fell ill. The employee was hospitalized for heat exhaustion.

An employee was stacking a tower and using a wooden board to align the tower. When the tower fell into place, the employee's left middle finger was crushed under the wooden board and the weight of the tower (about 4,000 pounds). His left middle finger was amputated at the first knuckle.

An employee was at a site where a communications tower was being installed. The employee was tying rebar when he became ill and then fainted due to a heat-related illness.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was working on a ladder that was placed on a paved driveway at the side of a house. The ladder slipped out from under him and he fell to the ground, sustaining fractures to his right elbow and left wrist.

On July 14, 2025, an employee became ill during work. He was hospitalized for a heat-related illness.

An employee was completing a fiber splice in a manhole. The lid of the manhole crushed the employee's left ring finger, causing the amputation of the fingertip at the first joint.

An employee was removing aerial service wires from an extension ladder when he fell 14 feet from the ladder. The employee sustained fractures to his pelvis, right hip, and ribs.

An employee fell while descending a ladder, landing on the ground and suffering a broken heel and lower leg.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
