An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was walking in a warehouse when he slipped and fell. His right leg went forward, his left leg buckled, and his left knee struck the floor. He suffered a dislocated kneecap and was hospitalized.

An employee was exiting a store when a steel gate, used to lock and protect the store, fell onto his right shoulder. He suffered a broken right humeral neck, as well as three broken right-side ribs.

An employee lost his balance and fell while entering a store office. His right side and back landed on cases/boxes and he suffered injuries to the T7-T11 vertebrae in his back. He was hospitalized and required surgery.

An employee was bringing extra bottles of liquor up from a cellar to the first floor when they lost consciousness and fell down the staircase, striking the concrete wall at the bottom of the stairs. The employee suffered a head injury and a collapsed lung.

A temporary employee was operating a cherry picker forklift with two pallets on the forks. While traveling backward, one of the pallets hit a storage rack. This pallet then pinned the employee's right foot against the rack resulting in amputation of the little toe.
