An employee slipped on a patch of black ice on a walkway and fell, suffering a broken knee. She was hospitalized, requiring surgery.
An employee had just returned to a boning line and was putting on her plastic cover while a co-worker was operating a handheld ring knife to cut hams on the line next to the employee. The knife struck the injured employee's right hand resulting in lacerations to her ring and index fingers and an avulsion to the middle finger. She was hospitalized.
An employee was troubleshooting a vibration and noise issue on a slicer. He manually rotated the idler pulley when his right index finger went through a cleaning hole in the pulley, causing it to get caught between the pulley and a bearing block. The employee suffered a partial fingertip amputation.
An employee was preparing to re-assemble a conveyor extension. She lifted the conveyor extension to place the arms in the bracket and lost control of the extension, causing the weight of the conveyor extension to shift and pinch her right ring finger against the conveyor frame.
