An employee was approximately 5 feet up on a ladder while drilling a hole for cable installation. A wasp attacked him. As he was coming down the ladder, he landed flat on his feet on the ground, and his right leg was twisted. The employee sustained a fractured tibia.

An employee was working from a ladder, pulling a cable line through a wall at a customer's house. He lost his balance and jumped off the ladder, landed on the ground about 7 feet below, and suffered injuries to both ankles including a fracture to at least one. The employee was hospitalized.

On June 16, 2025, at 4:30 p.m., an employee was ascending an 8-foot A-frame ladder to look inside an attic opening of a customer's garage. The employee was three rungs from the top when he fell 5 feet to the ground, resulting in a displaced fracture of his femur in the back of his knee. The employee was hospitalized and required surgery.

An employee was knocking on a customer's door when two dogs from a neighboring house attacked them, resulting in bite wounds that required hospitalization.

An employee was using a pocket knife to cut cardboard boxes. They were folding the pocket knife to put it away when it amputated their fingertip.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee had just finished pumping water from a manhole on one side of a two-lane street to the ditch on the other side of the street. While the employee was picking up the hose, a passing vehicle contacted the hose, which either pulled the employee to the ground or pulled the pump into the employee's leg. The employee suffered a broken left femur and was hospitalized.

An employee was closing a hand hole lid when the lid fell on the employee's fingers. The employee's left middle fingertip was partially amputated.

An employee was doing work in the field during a hot day, which included moving and setting up a 28-foot extension ladder and climbing the ladder to remove and attach cable lines. The employee became ill and was hospitalized with severe dehydration and heat-related illness.

An employee was working on a telecommunications strand and fell from a ladder. The employee was hospitalized for a spinal fracture and head trauma.

Employees were working to secure a low-hanging feeder. An employee was elevated in a bucket at a height of approximately 12 feet and was pulling the lashing wire. The lashing wire flipped up and struck the lower phase (120 volts) of power that was 2.4 feet above the feeder and strand. The employee sustained electrical burns to their chest, both hands, and the right forearm.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
