An employee was approximately 5 feet up on a ladder while drilling a hole for cable installation. A wasp attacked him. As he was coming down the ladder, he landed flat on his feet on the ground, and his right leg was twisted. The employee sustained a fractured tibia.

On June 16, 2025, at 4:30 p.m., an employee was ascending an 8-foot A-frame ladder to look inside an attic opening of a customer's garage. The employee was three rungs from the top when he fell 5 feet to the ground, resulting in a displaced fracture of his femur in the back of his knee. The employee was hospitalized and required surgery.

An employee was knocking on a customer's door when two dogs from a neighboring house attacked them, resulting in bite wounds that required hospitalization.

An employee was using a pocket knife to cut cardboard boxes. They were folding the pocket knife to put it away when it amputated their fingertip.

An employee was on an extension ladder to cut a high-tension midspan cable when it retracted, causing the employee to fall approximately 15-20 feet to the ground below. The employee sustained multiple injuries to the head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was standing on the surface of a steel beam about two feet wide. He slipped and fell backward, landing on the concrete ground about 20 feet below. The employee sustained a broken pelvis.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

An employee was working on a platform, raised 6 to 8 feet in the air, to get something off a rack. The employee fell from the platform to the ground and sustained a head injury and injury to multiple body parts.

An employee was preparing to unjam an auger shaft while standing on a forklift-elevated, job-made platform. The employee's wrench slipped off the equipment, causing them to lose their balance and fall. The employee landed on the ground 14 feet below and suffered fractures to the orbital bone, left elbow, and lower back.

An employee was adjusting a cable reel that was loaded onto the forks of a forklift. The reel shifted and caught the employee's left index fingertip against the backrest, amputating it.

An employee was on an extension ladder, about 15-18 feet high, while spraying a wasp nest on a telecommunications pole. He fell to the ground and sustained fractures to his hip, requiring hospitalization.

An employee was moving a pan through the pan building and was struck by the superstructure of a crane, resulting in a fractured hip.

An employee was pulling inner duct from one handhole to another. The shackle on a winch cable contacted the cable sheave's roller block. The employee released tension on the winch cable and lifted the cable to guide the shackle over the lip of the handhole. As he re-engaged the winch, his left index fingertip became caught between components in the winch's retrieval wheel. The fingertip was amputated between the last knuckle and the nail.

An employee had been laying cable in the sun for a full work day and later became unconscious. The employee was hospitalized for heat stroke.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

Two employees were working to change a tire on a mobile slasher saw. The slasher weighs around 2,000 pounds and is towed. Employee 2 was lifting the saw while the injured employee was placing a block of wood under the frame for support. The slasher then lowered onto the block and the injured employee's left thumb was crushed between the slasher frame and the wood block, leading to an amputation at the first knuckle.

An employee was using a metal shear when it amputated the tip of his right index finger.

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

An employee was standing on a step stool, removing the nuts and bolts from the frame of a solar panel that was being replaced. The employee's cheek made contact with a connector with damaged insulation. The employee was shocked, briefly lost consciousness, and fell to the ground, suffering an injury to the left shoulder.
