On June 9, 2023, an employee opened the radiator cap on an off-road truck. Hot steam came out and burned the employee's neck, chest, and upper arms. He was hospitalized.

An employee was using a bulldozer to smooth out a windrow of dirt near a pit used to bury dirt. The ledge of the pit collapsed causing the bulldozer to fall into the pit. The bulldozer rolled over onto the top of the cab and came to rest at the bottom of the pit. The employee sustained a concussion, a deep laceration to the head, and bruising to the ribs and shoulder.

An employee suffered muscle injuries and body cramps due to heat exhaustion while working at a jobsite.

At 2:39 p.m. on November 5, 2018, an employee was putting a metal storm drain grate (which had been removed for cleaning) back in place. It pinched a finger on his right hand, causing an amputation that left bone visible. He was hospitalized and underwent surgery.

An employee was using a machine to fill bags with birdseed. The employee was clearing a stuck bag under the machine when the bag cutter amputated the employee's fingertip.

A temporary employee was helping an operator remove jammed laminations from a press machine when their right ring finger got pinched between the laminations and the brake of the chute, resulting in a fingertip amputation.

An employee had just serviced a radiator on a concrete machine. During the final check on the machine, the employee was adjusting a hose when a fan caught his right index finger. The finger was amputated at the second knuckle.

Two employees were using a water hose to unclog a heart and liver separator machine. The machine was turned on while one employee's hand was inside it, and an auger under it amputated the tip of his right index finger. He was hospitalized.

An employee was cleaning dust from a collector. His left hand was caught in a rotary valve, which amputated his left index and middle fingers.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee fell while stepping off the tailgate of a pickup truck. The employee landed on the ground and suffered a broken left fibula and tibia.

On October 7, 2025, employees were conducting trenching operations for the installation of a new sewer line system. The excavator's bucket was detached and put inside the trench as it was being exchanged for a different-sized bucket. An employee entered the trench with a shovel to move some dirt located at the front end of the pipe. The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.

Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.

An employee was lifting a piece of 30-inch line stopping equipment using a jack screw. A lifting eye weld snapped and the load dropped. The end of the jack screw then landed on the employee's heel and crushed it. The employee was hospitalized.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
