Final narrative
On April 4, 2022, at 9:00 a.m., the injured employee was training another employee on how to adjust the deck height on a gas-powered push mower. A switch was engaged and caused the mower blade to strike the injured employee s left hand causing an amputation to the middle finger and a laceration to the ring finger. The laceration required stitches and the middle finger was reattached.
Employee was collecting shoes for her customer in the shoe stockroom when she tripped over a fitting stool sitting on the floor. She fell to the floor, fracturing her right ankle. She was carrying four boot boxes and could not see directly in front of her.
Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.
