Employees were attempting to correct an offset on a steel frame using a scale weight and a forklift. The forklift operator was setting the scale weight on the steel frame while the other employee was holding the steel frame. The employee holding the frame had their left little finger pinched between the weight and the steel resulting in amputation of their fingertip.
An employee was preparing to unload the contents of a tanker. He climbed the tanker ladder to cut off the seal and loosen wing nuts to allow the tanker to pump off. His hand slipped and he fell 8 to 10 feet to the ground and sustained a fractured left arm and three broken left ribs. Has was hospitalized and had surgery.
