An employee was climbing down a ladder from a roof. The employee fell from the ladder to the ground and suffered a broken hip.

An employee was driving a flatbed delivery truck loaded with construction materials (e.g. lumber) to a residential construction job site. The employee drove into the respective subdivision and past his destination. The employee stopped on the roadway, place the vehicle in reverse, and attempted to drive back to the destination. The vehicle stalled and the employee jumped from the cab of the moving vehicle resulting in a broken ankle.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was operating a punch press when he sustained injuries to his left index and middle fingers, resulting in the amputation of both fingertips.

An employee suffered a back injury while lifting a door weighing about 45-55 pounds.

An employee was tightening straps for beams (each are 90 pounds, 10-feet-long) on a semi-trailer when the beams became unstable and fell off the trailer and onto the ground and the employee's right foot. This resulted in the surgical amputation of the two little toes and half of his middle toe.

On August 17, 2023, an employee was operating an order picker that was carrying empty pallets. The employee went to avoid a pallet in his path when the vehicle collided with a rack, causing the pallets to fall from the vehicle. His leg was pinned between the order picker and the stack of pallets, resulting in leg fractures.

A forklift operator fell off the forklift and the forklift ran over his leg. The employee was hospitalized with head and neck injuries.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
