An airline catering truck driver had returned to the facility after completing his assignments and was feeling ill. He was hospitalized for heat related illness.

An employee was walking between rows of staged catering carts that were near a pallet of wet ice when she slipped on a plastic bag. The employee fell onto the floor and sustained injuries to the back of her head, right arm, and back.

An employee fell forward while walking down a hallway. Her elbow struck a raised curb at the side of the hallway and was broken.

An employee was pushing two equipment trolleys. As she pushed the carts through a door, her foot got caught under the door and she fell. She landed on her right side, suffering injuries to the right foot and hip.

An employee was exiting the back of a catering truck. The employee slipped and fell to the ground, suffering an injury to the right elbow.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

An employee was sitting on a milk crate at the back of a loading dock while talking to another employee. She noticed the other employee began shaking. She went to prevent the other employee from falling when she fell off the dock to the concrete ground, resulting in multiple body injuries that required hospitalization.

Two employees were working in the kitchen area of a food trailer. There was a sudden burst of fire in the kitchen, and both employees were engulfed by deflagration of gas which oriented from a propane-powered griddle. They suffered first-, second- and third-degree burns to their head, face, arms, and legs.

An employee loaded his truck. The dock plate was stuck under the carrier dolly and he could not lift the dock plate from the side while standing on the dock. The employee entered the truck and pushed the dolly away from the dock plate. This allowed him to lift the plate from inside the truck. While lifting the plate, his body shifted forward, and he fell between the dock and the truck, resulting in a fractured left wrist.

The injured employee had been conducting food service activities. He was stabbed twice in the chest and three times in the back by another employee using a pocket knife. The employee was hospitalized.

On April 2, 2025, an employee was reassembling a food-sealing machine when the machine activated and a metal plate pinched their right thumb tip, amputating it. The machine was not locked out/tagged out at the time.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
