An employee was lubricating a chain and sprocket on a corrugator splicer. He lost his balance, and his right middle finger came into contact with the chain and sprocket. The fingertip was amputated at the nail bed.

An employee was pushing a hand cart. When he crossed the T-cart aisle with the hand cart, the hand cart contacted a T-cart, pinning his left leg between the cart and the wall. The employee sustained a broken left leg.

An employee was servicing the #128 Rotary Die Cutter. The machine was de-energized and locked out. The employee was using a timing wrench to adjust a bar and belt in the machine. As the employee was twisting the wrench, his finger got pinched between the bar and another part of the machine. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
