An employee weighed and placed a drum of material onto a pallet and released the drum grabber fixture from the drum. As the employee began walking and repositioning the hoist with the remote control pendent, the hoist released from the beam and struck a drum causing impact with the employee's left leg. The employee suffered a fractured left leg and foot.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee was using a box cutter when it cut the tip of his left thumb resulting in a fingertip amputation.

Once it malfunctioned, the molten metal bled into the water line and started spraying into the area where the employee was located. The employee was hospitalized to treat burns to her arm, chest, and mouth.

An employee was loading components into fixtures on an automated CNC Line. He was moving a J-Hook lifting apparatus by hand from its storage location to the raw material area in order to pick up a piece of steel. The employee had the crane remote and the neck of the J-Hook apparatus in his right hand. When he went to set the J-Hook down on the floor, he placed his left middle finger inside of the lifting loop of the magnet. As he manually lowered the J-Hook to the floor by the neck, it fell. As the neck dropped to the floor, the magnet shifted and the lifting loop of the magnet retracted into the bracket of the J-Hook, fracturing and amputating the fingertip.

An employee was operating a two-handed punch press. When the foot pedal was actuated, the machine cycled and came down onto his left hand causing severe lacerations to the index, middle, and ring fingers to the bone.

An employee was cutting material on a band saw and amputated his left ring fingertip.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
