An employee was moving bar stock on the conveyor for a vertical bandsaw. The conveyor actuated, and his left index finger was crushed between a conveyor roller and the piece of bar stock he was working with. The fingertip was medically amputated.

An employee was handling metal material and suffered a hernia.

An employee had just lowered a spreader bar using a crane. When he removed the hook and raised the crane, it clipped the spreader bar and caused it to tilt over, landing on his left foot. The employee was hospitalized with a left foot fracture.

An employee was making adjustments to a limit switch located inside the slide gate electrical box. The motor actuated and his hand was caught between the chain and the sprocket. The employee sustained a laceration to the right hand that required stitches and hospitalization.

An employee was operating an overhead crane to move a coil to a staging rack. After setting the coil down next to another coil, the employee did not move the chain, which was caught between the two coils. When the employee went to move the crane with the lodged chain, the coil became unstable and began to fall. The employee stepped out onto the staging rack to stabilize the coil with his hands. The coil was too heavy and fell onto the rack and the employee's right and left legs. As a result, the employee suffered amputation of the left leg and a fracture to the right leg.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
