An employee was in the cab of the blender, preparing for well injection at a fracking site. The driveline disengaged from the transmission and started striking the cage that encloses the driveline. This created sparks and ignited a fire. The employee sustained burns around their eyes and cheeks and burns to both hands.

An employee was rebuilding a setting tool, attempting to seat the lower cylinder onto the cylinder head by tapping the setting tool onto the trailer floor. As the employee pushed down on the setting tool, the mandrel stroked in the setting sleeve and pinched and amputated their right middle finger.

As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt.
