An employee was rebuilding a setting tool, attempting to seat the lower cylinder onto the cylinder head by tapping the setting tool onto the trailer floor. As the employee pushed down on the setting tool, the mandrel stroked in the setting sleeve and pinched and amputated their right middle finger.
