The co-worker was operating a backhoe and the injured employee was on the ground maneuvering the mold to the desired location using a spreader bar attached to a chain that was hooked up to the front end of the backhoe. The chain broke and the employee's index finger became caught between the spreader bar and the concrete mold, resulting in a fracture that required hospitalization.
