An employee was helping a forklift remove a guard structure pole from a pole trailer. As the forklift was picking up the pole from the pole trailer, its forks tilted back. The pole began rolling backwards, and a bolt on it caught the inside of the employee's left safety glove as it rolled, twisting his arm in the process. The employee's left arm was broken, and he was hospitalized.

An employee was moving an articulating dump truck on a mat bridge when one of the tires caught the edge of the mat bridge and began to slide. The truck slid off the bridge and rolled onto its side. The employee suffered a punctured lung, fractured ribs, and a head laceration, requiring hospitalization.

An employee was hanging on a long line from a helicopter, clipping in travels on the arms of a transmission mono pole, when the helicopter maneuvered the employee into a three-phase distribution line. The employee suffered an electrical shock to the hip and buttocks, and later required amputation of the left arm from the elbow.

Cabela's

An employee was pushing carts into the chute area for customers to retrieve. The first of six carts struck the wall and caused the other carts to bounce back and strike the employee. The carts knocked the employee to the ground and she sustained a back injury.

RedGuard LLC

An employee was loading a portable storage container onto a dovetail trailer. This requires hooking up a bridle to the container and lifting the container enough for the dovetail to begin to slide under it. The employee reached and held the bridle lock to allow the trailer to move under it when the trailer moved backward instead of forward and caught his finger. The employee's right middle finger was amputated between the fingernail and the first knuckle.

Gulf Stream Marine

An employee was reaching over a small stack of steel billets on the ground to cut a strap. When the strap was cut, one of the billets (weighing 5,182 pounds) rolled down onto his right leg. He was pinned and suffered a broken tibia. The incident occurred while the employee was training.

Northern Electric Cooperative

On June 7, 2023, employees were unloading a box truck and rolling a reel of 2-inch plastic pipe to the back of the truck. As the employees were rolling the reel, the edge of the steel frame of the reel contacted the side of the truck. The injured employee s right little finger was caught between the reel and the truck, resulting in a laceration and fracture as well as a little finger amputation.

WDF, Inc.

Employees were using a dolly to remove a trash compactor unit from a residential home. As they began to transport it out of the room and to the outside, the dolly encountered uneven ground. This caused the dolly and the unit to move quicker than expected. The injured employee's left hand was then caught between a nearby concrete column and the compactor unit, resulting in fractures of the left ring and middle fingers that required amputation.

An employee was working to free a load by grabbing and shaking the live side of a tugger line. Once the load became free and progressed upward, the employee's hand was caught and pulled into a snatch block. The employee's right thumb was broken and lacerated. The employee was hospitalized and required a surgical amputation.

An employee was operating a drill rig when when their hand got caught between a 1/4-inch steel cable and a drill rod, resulting in the amputation of four fingers.

Elecnor Hawkeye, LLC

An employee was overseeing the operation of a rope being respooled onto the drum of a wire winding machine. He went to remove a piece of wooden reel from the rope. His glove got snagged and his right hand was pulled into the roller, pinching his fingers between the machine's roller and the rope. The middle and index fingers were crushed.

WEST AFFUM DEVELOPMENT CO LLC

An employee was visiting a patient's home to fit them with a wearable cardiac defibrillator. As he was walking from the front door to his car, a large dog broke free from a chain and attacked him. He was bitten on the forearm and fell. The patient was then able to restrain the dog. The employee was hospitalized with severe dog bites and required surgery.

An employee tripped over a junior I-beam about 4 feet long and 6 inches tall that was installed for circulating water valves and fell to the ground, resulting in a broken right-side rib.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

APM TERMINALS PORT ELIZABETH

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
