Final narrative
An employee was installing an I-beam vertically in a doorway to close up flat storage. The I-beam began to fall and the employee held onto it so it wouldn't hit another employee. His left index finger was pinched between the beam and a skid steer amputating the fingertip.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.
An employee climbed an aluminum extension ladder inside a grain bin to help remove a stirring machine. As the farmer began to lower the stirring machine from the top, one of the ropes broke, causing the stirring machine to strike the ladder and knock it out from under the employee. The employee fell to the base of the grain bin and sustained fractures requiring hospitalization.
An employee was assembling salads for packaging when her smock/apron got caught in the conveyor belt. The employee removed the ensnared smock and sprained her left shoulder. The employee was hospitalized.
Two employees were working on a roof that was approximately 18 feet above the lower level. The downspout for the grain bin (from a leg) was clogged and was removed by the two employees. While cleaning it, the injured employee slid off the roof and landed on their feet, fracturing their left ankle. Fall protection was not worn at the time.
An employee was loading a semi-trailer with corn. When climbing out of the silo, he slipped and fell onto the auger on the outside of the silo. His feet became entangled in the auger and were severed from the ankle down. A surgical amputation of both legs from the knees down was required.
An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.
An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.
An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.
Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.
An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
