Final narrative
An employee was going down ship-style stairs in a turbine building. The employee slipped, fell down the stairs, and suffered a broken lower right leg.
On 04/30/2021 at 12:20 a.m., an employee was assisting two co-workers in retrieving a piece of expanded metal located behind a stack of sheet metal. The stack of sheet metal contained 25 to 30 sheets of 4-foot by 10-foot sheet metal that weighed approximately 50 pounds each, or 1,250 to 1,500 pounds total. While the employee and a co-worker were manually holding the stack of sheet metal, they lost control of the stack and attempted to move clear of the falling metal. The employee's right foot was pinned beneath the stack as it fell, resulting in a foot fracture.
An employee was adjusting an energy-isolated industrial fan belt and pulley. The employee was manually rotating the motor and assembly when the employee's right middle finger was caught between the belt and pulley, lacerating and fracturing it.
An employee was cutting a 45-degree angle on a piece of 2x4 wood when the circular saw kicked back. The saw blade severed multiple tendons in the employee's forearm and penetrated the bone just above the wrist.

A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.
An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.
An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.
An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.
An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle.
