The tire was not yet inflated and the employee was pinned by the tractor. The employee sustained crush injuries to the hip, pelvis, and tailbone including several fractures to the pelvis.

An employee was working underneath a vehicle that was suspended on two jack stands when the vehicle frame broke. The vehicle fell onto the employee, resulting in a back fracture, rib fractures, and a punctured lung.

On June 17, 2023, an employee was checking a tire for leak. As he released the air brake, the semi-truck fell from the jack onto the employee's right arm and rolled forward. The employee sustained an arm injury.

A forklift was unloading a pallet jack from a rental truck when the pallet jack fell off the raised forks and struck an employee as she was working to steady the load. She sustained a right thigh fracture along with scrapes and bruises.

An employee was on a 20-foot ladder cleaning tile on the side of a building. The ladder slipped and he fell to the ground. He landed on his feet, resulting in hospitalization with fractures to both feet.

An employee was putting away stock in the aisles and began driving his order picker down the aisle with a pallet of items loaded on the forks. The pallet struck an upright of the warehouse racking and was pushed toward the employee, causing him to fall. The front edge of the pallet drove his right leg into the front panel of the order picker, fracturing his right leg above the ankle.

An employee got out of his truck and closed the door when his right hand got caught in the door, resulting in a broken ring finger and an amputation of the middle fingertip that required hospitalization.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
