An employee was working on a second-level roof area. He jumped down to a porch roof, fell through it, and struck the ground. He suffered broken bones and was hospitalized.

At 1:00 p.m. on July 26, 2022, an employee was three rungs from the top of an 8-foot ladder. The ladder shifted and he fell to the ground, breaking his left leg.

An employee was guiding a skid steer when it ran over his leg. The leg was broken.

On February 21, 2022, an employee was using a tractor to push snow off and away from a dolly axle. The employee had shoveled most of the snow when they got off the tractor and positioned the bucket so they could slide the axle into the bucket. As the employee was trying to tip the axle into the bucket, their foot slipped on the ice and snow. When the employee fell, the I-beam on the axle crushed their hand against the side of the bucket, resulting in an injury to their finger.

At 8:30 a.m. on December 23, 2021, an employee was unloading roof steel from a cradle, suspended from a telehandler boom. A cable became unhooked from the boom and the cradle opened up; the steel slid from the cradle, traveled about 6 feet down the roof, and pinned the employee against the chain on the lift truck. The employee suffered broken ribs and was hospitalized.

An employee was performing finishing work on a residential driveway when they began to experience body cramps and were hospitalized for heat stress and dehydration.

An employee was staking tomato plants on a farm. The employee began to feel sick and collapsed, suffering from dehydration and heat shock.

An agent was participating in SWAT team selection and was performing various physical fitness skills including running and exercise intervals. The agent experienced dehydration and a muscular injury that required hospitalization.

On September 26, 2023, an employee was delivering packages when he began to feel ill with a pain in his side. He was hospitalized for heat exhaustion and dehydration.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was standing on a multi-purpose ladder installing head flashing over a window. Their feet were approximately 6 feet off the ground and three rungs from the top. The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee fell through a hole in the ceiling and suffered injuries to his side and right arm. When he fell, a piece of concrete fell with him and struck his arm. He was hospitalized.

An employee was laying glue on the corner of a three-story commercial flat roof to apply new thermoplastic polyolefin roofing material. He lost his balance and fell at least 35 feet to the concrete ground below. The employee sustained fractures.

An employee was installing roof tiles. The employee fell about 6 feet and suffered a back injury.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
