An employee was walking around chairs when they slipped/tripped and fell, resulting in hospitalization with a fractured hip.

An employee was entering a building and fell, resulting in a fractured right hip.

An employee was exiting the building following the completion of their shift. The employee slipped and fell on a sidewalk that was covered with snow and ice. The employee sustained three fractures to their right ankle. The employee was hospitalized and required surgery.

The injured employee was pulling supplies from primary storage for distribution. A second employee entered the room and spilled the contents of two 1-gallon jugs of reagent methanol (95%) onto the employee.

An employee was carrying items from their car into their office and fell in the parking lot after stepping on a patch of grass, resulting in a fractured left hip.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was walking down a hallway when they stepped on a small puddle of liquid and fell backward onto their left leg, resulting in a broken left femur.

An employee was on a 10-foot ladder replacing a fan motor on an HVAC system. While removing the guard to facilitate maintenance, the guard slipped. As the employee tired to grab it, their left hand fingers were caught between the belt and pulley resulting in a partial amputation of the little finger and fractures and lacerations to the middle and ring fingers.

An employee was using a fire escape when he slipped on the stairs that were wet due to rain. The employee fell down several stairs, hit his head, and injured his right hand.

Three employees were working on a steam pipe. They had depressurized the line and cut it. As they were preparing to replace the broken pipe section, one of them was struck by pressurized hot water and/or steam, which burned his right arm, chest, and back. He was hospitalized.

An employee was pushing a cart when they stopped to assist someone. As they turned around, they fell backward down the stairs with a box and hand truck landing on top of him. The employee was hospitalized and sustained rib fractures, a hematoma, and a compression fracture/spinal injury.

An employee was closing a gun locker in his automobile trunk when his left middle finger was partially amputated by the lid.

An employee was ascending a flight of stairs after exiting their vehicle in an adjacent employee parking lot. The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

A mail handler was moving BMCs on the loading dock when they fell from the loading dock to the concrete ground below. The employee was hospitalized with head trauma.

An employee was on an A-frame ladder while preforming demolition of a ceiling. A section of HVAC ductwork fell on the employee and knocked them to the ground approximately 12 feet below. The employee was hospitalized with a concussion and lacerations.

On the morning of July 24, 2025, an employee was conducting synthesis of a highly toxic experimental chemical (nerve agent) in a chemical fume hood. The operation was completed at approximately 11:30 AM. The employee was exposed to the chemical and began experiencing symptoms of illness/poisoning around 1:30 PM and was hospitalized.
