An employee was standing over a hydraulic cylinder, unpinning a saddle clamp that held the cylinder in place. When the clamp came free, the employee fell to the deck and suffered a broken knee and wrist.

Independence Contract Drilling, Inc.

On August 20, 2025, at 8:00 a.m., two employees were removing a suction module on a mud pump. Three bolts were sheared off and the fourth bolt was frozen, making it difficult to remove the module. The employees used a 5-foot pry bar inserted in the bolt hole to free up the module while a forklift bumped and lifted the module using a two-part sling. When the forklift bumped the module, the module came free, causing the pry bar to jump and pinch the injured employee's left little finger against the pulsation dampener, which resulted in a partial fingertip amputation.

An employee was unpinning a beam and removing it from the pinned position. As he pulled it from the pad eyes, the beam fell approximately 18 inches to the ground. The employee's right middle finger was caught between the beam and the ground, resulting in a fracture to the distal portion of his finger that required surgery.

A casing swage (weighing approximately 50 pounds) was being installed to a joint of casing in order to circulate drilling mud through the casing. The swage was approximately 40 feet in the air when it came free and fell striking an employee in the hard hat. The employee sustained a skull fracture.

An employee was changing the attachment on the front of a telehandler forklift. His finger was caught between the attachment and the forklift. He suffered a partial amputation to the finger.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

A service crew was pulling a pump and motor from a well. After breaking a joint of pipe loose from another joint, an employee grabbed the chain tongs to prevent the pipe from spinning in the well, so the crew could continue taking it apart. The bolt that attaches the chain to the handle of the chain tongs broke, causing the chain to swing around and strike the employee's lower right leg. The chain severely punctured his leg and fractured his tibia.

An employee was working to remove a stuck fish tool from a drill shoe. Their thumb was struck by a hammer, resulting in amputation.

An employee arrived on location and loaded a blow-out-preventer (BOP) on a 1-ton flatbed truck. While standing on the flatbed securing the the BOP, the employee stepped into a void/hole on the flatbed surface. This caused him to lose balance and fall off the truck bed with his leg still caught in the hole. As a result, he sustained fractures to his left leg.

An employee was rigging up a location. He was using an Allen wrench to free up rotation and start the drawworks engine manually. The engine rotated and his left little finger became caught between the Allen wrench and the engine. The employee sustained an amputation to his little finger and fractures to his wrist.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
