An employee was closing a valve on one hopper and opening the valve on another. While the employee was stepping over a product feed hose, her foot was caught. She fell to the concrete floor, suffering a fracture and dislocation of the right elbow.
An employee was performing housekeeping duties. She reached under a piece of equipment to clear some product when her left hand got caught in a motor belt, resulting in a partial amputation of the index finger.
