Final narrative
Two employees were outside a drum, vacuuming out leftover desiccant through a 6-inch window. A flash fire occurred and the employees sustained first and second-degree burns to the face, abdomen, arms, chest, fingers, and wrists from ethylene, propylene, and hydrogen. Both employees were hospitalized.

An employee was running a line moling machine to clean the inside of pipe. When the employee went to pressure up the system using a 20-k water line, water from the hose injected into the pinky finger on his left hand.
