Final narrative
An employee had climbed onto the rebar lattice for one of the walls of a concrete foundation and was tying rebar together using lengths of wire. While transitioning from one location to another, the employee slipped and fell about 8 feet from the rebar lattice to the concrete foundation below resulting in a laceration to the head. The employee was wearing fall protection but it was not connected to an anchoring point at the time of the fall.
An employee was putting tires on a motor grader. After airing the tire up, the employee noticed that the O-ring was protruding from the seal, indicating a bad seal. The seal ruptured and the employee was struck by rubber and flange from tire.
