Final narrative
An employee was on an extension ladder using a pneumatic grinder to prepare a pressure vessel for welding. The ladder slid out from underneath him and the employee fell approximately six feet to the ground, landing on the ladder. The employee sustained a compound fracture to the right lower leg.
An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car.
