Final narrative
An employee was pulling a customer order. The employee's sock was caught on a skid and the employee tripped over a pallet, fell to the concrete floor, and suffered a broken hip.
On or about April 9, 2015, at approximately 1300 hrs. an employee suffered the amputation of the distal phalanges on the right hand pinkie finger. The employee was attempting to load a bundle of angle iron onto a vehicle when the load shifted in his hands and pinned his right hand to the structure of the vehicle.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
An employee was operating a reach truck and was coming around corner when the vehicle struck a warehouse rack. The employee's left foot got caught between the forklift and the rack. The employee was hospitalized with fractured toes.

The employee moved the forklift forward and then fell to the floor, sustaining injuries to their leg and foot.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
