A crane operator was removing lifting clamps from a steel girder. As the operator was raising the hoist to remove the clamps, the girder titled and fell pinning him between the girder he was removing the clamps from and a girder behind him. Both of his femurs were broken.

An employee was cleaning a steel girder in a blast booth when the supply line got kinked and twisted near the nozzle. The employee lost their grip on the nozzle and were struck in the head, face, and neck by blasting media. The employee's chin area was impacted causing abrasions and a puncture wound that resulted in hospitalization.

Regal Industrial Corporation

On August 13, 2015, an employee was operating an overhead crane to move a steel beam that was approximately 4' by 15' and weighed 4,000 pounds. The beam was sitting in the I position and the employee was using beam clamps to move the beam to horses. The employee's right middle finger got caught between the beam clamp and the beam flange. The top portion of the employee's finger was sheared and had to be surgically amputated.

An employee was checking the function of newly installed actuators when they were struck by a pressurized release pipe. The employee suffered fractures to the orbital socket and shoulder, requiring hospitalization.

An employee was removing chains from a steel tube. A sorting hook attachment was caught on the staging area cross member. When he raised the chains, the sorting hook snapped back and struck the employee's face causing a fractured jaw.

An employee was in the sawmill basement using a push broom and shovel to clean wood chips and sawdust. The employee's right arm was struck by a wooden board between the wrist and elbow, resulting in a fracture.

Southwest Galvanizing, Inc.

An employee was cutting steel channels from a hanging rack that was being supported by a forklift. When they were finished, the forklift started moving the rack out of the area and a piece of material snagged one of the rack legs, causing the rack to tilt backward. The rack slipped off the forklift and struck the employee on his right side/lower back. He sustained a fractured pelvis and was hospitalized.

Valmont Industries, Inc.

An employee was working to remove waste wire between two boat rack/spreader bars (10 feet in length, 8 inches wide, and 16 inches tall, with a center lifting point for a crane hook) in the hanging department. One boat rack was leaning on another boat rack. The employee reached between the boat racks to move the leaning boat rack and remove the waste wire. The leaning boat rack shifted and the employee's left index finger was pinched between the two racks (weighing approximately 600 pounds each). The employee sustained an abrasive amputation of the left index fingertip from the nailbed up.

Steel Goode Products, LLC

An employee was performing maintenance on a coating machine and the belt cover guard was removed to repair the belt. The moving belt caught the employee's fingers and pulled them between the belts, injuring three fingers on the employee's left hand. The fingernails were removed from their little and middle fingers. The ring finger was amputated at the first knuckle.

Howmet Aerospace

An employee tripped, fell to the floor, and suffered a broken right radius. The employee was hospitalized, requiring surgery.

V & S TAUNTON GALVANIZING, LLC

An employee was stacking steel grating. The grating moved and their right ring finger got caught between grating. The last digit of the ring finger was amputated.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
