Two employees were conducting maintenance on a pipe that was leaking on the production floor. While repairing the broken line, a cap broke off, releasing 12 lbs. of steam into the room. While evacuating the room, an employee slipped and fell on water. He sustained a dislocated shoulder and burns to 20% of his body.

Carestream Health, Inc.

An employee performing maintenance on a forklift was removing the hydraulic cylinder from the mast, which was elevated on spacer blocks. When the cylinder was removed, it struck the spacer block, causing the inside rail to lower. The employee's left hand was pinned between the inside and outside rails of the forklift, resulting in a crush injury and a laceration between the knuckles and wrist.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
