An employee saw a pallet fall off a forklift into a bin where it did not belong. He left his forklift and went over to the other forklift. He rode the forks of the forklift to a 6- to 8-foot high elevation and retrieved the pallet. As he was riding the forks down, his right-hand ring fingertip became trapped and was amputated between the mast and the rack of the forklift.
Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.
