An employee lifted a door above a basket stacking machine. As the employee went to unlatch the door to lower it back down, the door dropped and struck the employee's left middle finger, resulting in an amputation above the knuckle.

An employee was cleaning a freezer while on an A-frame ladder when they fell 8-10 feet and sustained a fractured rib and sacrum.

On February 27, 2022, an employee was moving a skid of ingredients with a walk behind powered pallet jack while walking backward. When he turned a corner, his right foot was pinned between the pallet jack and a concrete bollard. He suffered a broken right leg.

An employee was working in the layer cake department at the second stage icing machine. The employee went to remove a defective cake from the line when her finger was caught in the carousel conveyor and amputated.

An employee was placing a covering on the discharge duct of a sugar hopper. The employee's fingers contacted the motor blades in the sugar feeder valve and the employee's right index and middle fingertips were amputated.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was installing doors when one of the doors fell on him, fracturing his lower leg. The employee was hospitalized.

An employee (who had recently walked through water accumulated on the floor) slipped and fell to the floor. The employee suffered a broken right wrist and right elbow and was hospitalized, requiring surgery.

An employee was performing maintenance on a production line, requiring a testing and positioning phase for the whipped cream dispensers. The employee was positioning the dispensers using wrenches when one of the sensors was triggered by the pie pans coming down the line. The depositor heads lowered and pinched the employee's left ring finger, resulting in an amputated fingertip. The whipped cream dispensers were not guarded at the time and the line was not locked out/tagged out.

During a line changeover, an employee was rinsing a depositor with water. The employee was working to remove a piece of chocolate, stuck in the machine's roller, when the roller's moving parts caught his middle finger. He suffered an amputation to the fingertip with bone loss.

An employee was emptying a trash can into a dumpster. He tripped while turning around, fell over a rail onto the concrete floor, and broke bones in his back and wrist.

During a changeover, an employee was wiping down the dough chunker machine and the chunker closed on his left hand. The employee sustained crushing/laceration injuries, requiring hospitalization and surgery.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
