An employee was grabbing a pedestal fan. The employee's thumb went through the guarding and contacted the fan blade. The thumb tip was amputated.
An employee was operating a blender and feeding ingredients. He opened the door to clear the ingredients off the blade and his right hand contacted the blade resulting in a hand fracture and amputations to four fingertips.
An employee was descending a rolling staircase. The employee fell from the staircase to the floor below, suffering a broken right knee. The employee was hospitalized for treatment and surgery.
An employee was using an industrial size dust collector (vacuum) to clear out a packaging machine. The employee's hand became caught in a turning valve roll in the packaging machine, resulting in the amputation of the right index and middle fingertips. The machine was running at the time of the incident.
An employee was removing ketchup from a fixed pusher bar. The employee's fingernail was caught in the conveyor; the fingernail was removed, the fingertip was amputated, and the distal phalanx was fractured.
