An employee was using a pallet jack to load pallets into a trailer. The employee stepped off the pallet jack and the vehicle continued to move forward, crushing his leg between the pallet jack and a pallet. He sustained fibula and tibia fractures and two ankle bone fractures.

An employee was parking a reach truck in the charging area when his left calf was pinched against another reach truck. The employee was hospitalized with a laceration to the left calf.

An employee was driving a forklift when it struck something on the ground and slid into a safety pole used to protect doors. His foot was crushed between the forklift and the pole, resulting in a fractured foot.

An employee was requested by another employee to assist with a dock leveler that was not operating. The employee went behind the vertically stored dock leveler to attempt to troubleshoot the issue. The vertically stored steel dock plate fell on the employee's lower body, crushing them between the steel dock plate and the trailer floor. The employee sustained a fractured pelvis, bladder injuries, and nerve damage.

An employee was about 5 feet up an un-extended extension ladder, working on the HVAC system of a walk-in cooler. The ladder kicked out backward from beneath the employee, who fell to the ground and suffered a broken left forearm. The employee was hospitalized.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
