An employee was using a battery-powered scissor lift to load a trailer. The scissor lift shut off halfway up the ramp leading into the back of the trailer, drifted backward, and fell off the side of the ramp. The employee rolled with the scissor lift, jumped off, and was crushed. The employee suffered an open fracture to the left elbow and was hospitalized.

An employee was operating a forklift to remove a pipe from the lower section of a pipe rack. Once the employee had the pipe on the forks, the employee parked the forklift and walked to the load to center it. Another pipe then shifted off the forks and fell, pinning the employee between the pipe and an ingot up ender behind him. The employee sustained a tibia and fibula fracture.
