An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was repairing and testing a side-by-side utility terrain vehicle. The vehicle rolled over, and the employee suffered two broken ribs.

An employee was lying on the ground behind the drop axel of a semi-truck while checking the brake chambers when the drop axel lowered due to the engine shutting off. The axel struck the injured employee and pinned them against the floor, resulting in broken ribs and a swollen eye.

An employee was ejected from a golf cart that flipped over while going up an incline. The employee was hospitalized for pelvic fractures.

An employee was on a ladder removing a pin that was stuck on a garage door when the door released and knocked them off of the ladder, causing them to fall 10 feet. They sustained fractures to two ribs, a scapula, and their skull.

A fuel truck operator was crawling under a fuel truck to put down an absorbent pad, due to a fuel leak. The employee was caught in a rotating shaft under the truck and sustained a broken ribs, a fractured vertebra, and a right shoulder injury.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
