An employee was operating an asphalt grinding machine when a branch got in the way of the machine. He crawled on top of the machine to cut the branch but it snapped back and knocked him off the equipment. He fell approximately 10-12 feet to the ground below, resulting in fractures to both wrists and forearms, and a left knee fracture.

A temporary employee was on a ladder while cutting conduit on a new building using a reciprocating saw. They sustained an electrical shock from a live electrical cable. The employee fell backward approximately 6 feet to the ground, resulting in hospitalization with a fractured heel that required surgery.

An employee was working on the ninth position of a 12-foot ladder, installing a new light fixture in an existing building. The employee sustained an electrical shock, lost his balance on the ladder, and fell. He sustained a fracture of the lower back and bruises on his hand and foot. The fracture required surgery.

At approximately 10:00 AM, employees were conducting asphalt loading operations. While securing a hose atop a truck, an employee encountered a malfunction involving a plugged hose. Pressure built up within the hose and suddenly released, causing the hose to fly off the truck. The hose contacted two employees and both employees fell approximately 7 feet. Employee 1 was on top of the truck and sustained three fractured ribs and a head contusion. Employee 1 was hospitalized. Employee 2 was climbing a ladder on the truck and sustained a fractured right leg. Employee 2 was not hospitalized.

An employee was working from a ladder, installing light fixtures. The employee was shocked and fell from the ladder to the floor about 6 feet below, striking their head and back. The employee was hospitalized with a laceration to the head and broken vertebrae.

An employee was repairing and testing a side-by-side utility terrain vehicle. The vehicle rolled over, and the employee suffered two broken ribs.

An employee was lying on the ground behind the drop axel of a semi-truck while checking the brake chambers when the drop axel lowered due to the engine shutting off. The axel struck the injured employee and pinned them against the floor, resulting in broken ribs and a swollen eye.

An employee was ejected from a golf cart that flipped over while going up an incline. The employee was hospitalized for pelvic fractures.

A fuel truck operator was crawling under a fuel truck to put down an absorbent pad, due to a fuel leak. The employee was caught in a rotating shaft under the truck and sustained a broken ribs, a fractured vertebra, and a right shoulder injury.

An employee was lubricating a motorcycle's chain. The chain and sprocket caught his fingers and amputated a fingertip.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
