An employee was cleaning a printing machine. An ink tray fell on the employee's finger, crushing it, pulling flesh off the finger, and fracturing the fingertip.

An employee was using a rag to wipe lubricant off the feed table of a flexo machine. The rag was pulled into the feed rollers, pulling the employee's hand with it. The employee sustained an amputation to the right index finger.

An employee was stepping in and out of a cardboard gaylord container through a cutout, retrieving small merchandise items from the container. While stepping out of the container, the employee tripped on the bottom of the cutout and fell to the concrete floor. The employee suffered a broken right hip and was hospitalized.

On July 30, 2019, an employee was scraping varnish away from a blade with a chisel. The chisel slipped off the blade and the employee sustained a tissue amputation to the right index finger that required sutures.

An employee was using a utility knife to cut corrugated cardboard. The knife cut off the tip of the employee's left middle finger.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
