On July 22, 2024, at 10:30 AM, an employee was responding to a pulp mill manufacturing process shutdown. Pulp stock overflowed through an engineered overflow pipe resulting in a buildup of stock in a bermed area. The stock was 140 to 150 degrees. The employee went to the bermed area to close an air-actuated process valve. While closing the valve, pulp stock entered the top of his boots and caused second-degree burns to both of his lower legs down to his feet. The employee was hospitalized.

An employee was removing a hard salt cake plug in a rotary dust valve on a precipitator in the utilities department. Their left ring fingertip was amputated by the rotary dust valve.

An employee was assisting three other employees to load a reel of paper into the saddles of an unwind stand when the springs compressed and the gap closed on the injured employee s finger. They sustained an avulsion to the left ring fingertip that was surgically amputated.

An employee was on the belted manlift when they fell to the floor, resulting in a collapsed lung and multiple fractures to the ankle, ribs, shoulder, and back.

On 7/9/2021, at approximately 2:15 p.m., an employee was attempting to leave their desk and got caught on the corner of the desk, causing a trip and fall. The employee fractured their left hip and was hospitalized for surgery.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

Tools and parts were being lowered to the ground from an elevated platform in a 5-gallon bucket. The rope being used to lower the bucket failed; the tools and parts fell and lacerated the left forearm and upper left leg of an employee on the ground.

An employee was tracking an infeed belt on a scrap hogger when their right arm was caught between the belt and a roller. The arm was broken and the employee's shoulder was dislocated. The employee was hospitalized.

An employee was working to clear a jam from a paper napkin folder. The ram that advanced the napkins pushed the employee's right arm into the side of the machine, causing several lacerations and a hairline fracture.

An employee was troubleshooting overflowing ink on a printer-slotter machine when their right ring fingertip was caught in a guarded roller, resulting in a partial amputation.

An employee was disconnecting wires in preparation to swap out a refiner motor when they contacted energized equipment and sustained an electrical shock. The employee was hospitalized.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
