On 12/18/16, an employee was walking up the basement steps while carrying a few packages of frozen meat when she missed the top step, lost her balance, and fell. When she attempted to brace herself from the fall, her right hand went though the lower tier window of the back door, severely lacerating her right wrist.

An employee was preparing a snack for a patient when the patient strangled the employee. She lost consciousness and was hospitalized.

An employee was choked by a resident, fracturing her trachea and requiring hospitalization.

An employee was choked by a patient while leaving a patient's room resulting in fractures to the throat.

An employee was assaulted by a patient (choking) resulting in a multi-day hospitalization.

An employee was cutting tomatoes for sandwiches when the knife amputated her left index fingertip.

An employee was transferring a pot of hot water and noodles from the stovetop to the sink in the kitchen. The employee fell and their leg was burned by the hot water.

An employee was coming into the facility to bring a note when they tripped on a carpeted mat and fell forward, hitting their head on the tiled floor. The employee sustained a subdural hematoma and a head injury requiring 30 sutures. The employee was hospitalized.

An employee was leaving a client's home when they slipped on black ice in front of the home, falling to the ground and suffering cranial fractures and a brain bleed.

An employee was walking out of the house when she fell down a flight of stairs. The employee was hospitalized with torn ligaments.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
