An employee was moving a parts carrier with an overhead crane. The employee was repositioning the hook for the overhead crane, which weighs approximately 50 pounds. The hook shifted and pinched the employee's left middle fingertip, resulting in an amputation.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
